How to Sign Up For Manifest MedEx (MX):

STEP 1: Sign the Contract

  1. (OPTIONAL) Schedule an appointment with IEHIO here to review the MX onboarding process and answer any questions you may have.
  2. Download the MX Participation Agreement here.
  3. E-mail a signed and dated copy of the contract to info@iehio.org or fax it to (951) 686-1692. Please do this within three days of dating and signing the agreement.

STEP 2: Schedule a Meeting with your MX Customer Success Account Manager to Upload a Patient Panel

STEP 3: Attend a Kick-Off Meeting with MX & Start Sharing Data!
